What to do before your appointment
Ask your providers to email records to hello@cccofnwa.com
Send us a copy of the front and back of your medical insurance card to hello@cccofnwa.com
Email any test results related to the craniofacial condition, including CT images or x-rays.
Complete and bring the new patient paperwork or arrive 15 minutes early to do so.
Please write down any questions you have. We want to make sure we answer all your questions.
Inform us of anything you need, or we should know before your appointment.

What to expect during your appointment
Bring to your appointment medical and dental records, photos, scans, and medication list.
During the first visit, the craniofacial team will meet with you and review your medical history and conduct any necessary exams.
In cases where additional testing is required for a diagnosis, you may need to see a geneticist.
The craniofacial team will explain your diagnosis.
The craniofacial team lead will discuss your treatment plan and options.
Remember: you will always have a say in your treatment plan.
Medical records taken at every appointment might include recording height and weight, photos, x-rays, and CT scans.
Might require additional specialists depending on your needs: Speech therapist evaluations, Orthodontic evaluations, Primary Care evaluations…
If you have a primary care provider, we will send over our clinical exam notes, diagnosis, treatment plans to your primary care doctor and to your team.
We will continue to stay in contact with your doctors throughout treatment.
Schedule appointments with geneticists or additional doctors.
